Most of the forms are available in the Church office, from your Sunday school teachers, or from the Refuge leaders. If you have questions about any of the forms, please give Daralyn Wallace a call in the Church office at 979-696-7000.

The medical release forms must be updated every year in January. We must have these on file for each activity that takes Youth away from the Church property. If you have not updated yours for 2011, please download it, fill it out, and take it to Daralyn Wallace in the Church office. Thank you!
